Liability Waiver and Release Agreement

1. Assumption of Risk I acknowledge and understand that participation in activities at The Sportz Factory, Inc., including but not limited to batting cages, training, and sports-related exercises, involves inherent risks of injury, including but not limited to sprains, fractures, concussions, or other serious injuries. I voluntarily assume all risks associated with participation.

2. Release of Liability I, on behalf of myself (or my minor child), hereby release, waive, and discharge The Sportz Factory, Inc., its owners, employees, trainers, agents, and affiliates from any and all claims, liabilities, demands, or causes of action related to injury, illness, or damages arising from participation in activities at the facility.

3. Medical Treatment Authorization

   In the event of injury, I authorize The Sportz Factory, Inc. to obtain medical treatment as deemed necessary. I understand that I am responsible for any medical expenses incurred.

4. Conduct and Safety Rules

   I agree to follow all facility rules and staff instructions. Failure to comply may result in removal from activities without refund.

5. Parental/Guardian Consent (For Participants Under 18)

   I, the parent/guardian of the above-named minor, acknowledge the risks and voluntarily consent to their participation. I agree to the terms of this waiver on behalf of my minor child.

By signing, I acknowledge that I have read, understood, and voluntarily agree to this waiver.
